ATTENTION NEW PATIENTS:
Failure to have your paperwork completed when you arrive at the office for your appointment may result in being rescheduled. We allow only enough time in our schedule for you to be evaluated medically. Out of respect for other patients that are also being seen that day we need to stay on schedule. This can only be accomplished by all new patients being prepared when they arrive.
If you are in need of assistance completing the requested information please arrive at the office at least 30 minutes prior to your scheduled appointment, that way our front office staff can provide you with the assistance you require while still allowing you to be seen at your scheduled time.
